届出種類 / Category
本学で債主登録したことが無い場合は「新規」を選択してください / Please select “新規” if you have not registered your bank account information in our Institute.
学生証に記載のとおりに氏名を入力してください。姓と名の間に全角スペースを入力してください。ミドルネームがある場合はその都度全角スペースを入力してください / Enter your name on your student ID card in full-width characters with a FULL-WIDTH space between your first name and last name. If you have a middle name, enter a FULL-WIDTH space each time.
姓と名の間に半角スペースを入力「しないで」ください / Enter your name in half-width katakana WITHOUT a half-width space between your first name and last name.
半角入力。現在修士課程の学生は修士課程の学籍番号を入力してください / Enter half-width numbers. If you are a master’s degree student, enter your master’s student ID number.
本学理工学系の修士又は学士課程の学籍番号がある場合は入力してください / Enter the student ID number of the master’s or bachelor’s of our Institute for the science and engineering fields, if any.
半角入力 / Enter half-width numbers

住所 / Address

住民票記載の住所を入力してください。
Enter your address registered on the resident record (住民票 Jūminhyō).

全角入力。最大15文字。「市」等まで含めて入力 / Enter the name of your city/wards/towns/villages including 「市」 etc. up to 15 FULL-WIDTH characters in Japanese.
全角入力。最大20文字 / Enter the street name up to 20 FULL-WIDTH characters in Japanese.
全角入力。最大25文字 / Enter the building name and room number up to 25 FULL-WIDTH characters in Japanese.

口座情報 / Account Information

入力前に支援学生本人の通帳やオンラインバンクのスクリーンショットをご用意の上、通帳等を確認しながら口座情報を入力してください。
Before entering your account information, please have your bankbook or a screenshot of your online bank and enter the information into this form while verifying the documents.

ゆうちょ銀行の場合 / For Japan Post Bank Account Holders

支店コード・口座番号は通帳の青丸で囲った箇所の情報を入力してください。
Enter the branch code and account number in the area circled in blue on the bankbook.

ゆうちょダイレクト＋(プラス)の場合 / For Japan Post Bank Account Holders Who Are Not Issued Paper Bankbook

通帳が発行されないタイプの場合は「通帳イメージ」のスクリーンショットをご提出ください。

半角入力。ゆうちょ銀行は「9900」を入力してください / Enter half-width numbers. Enter 9900 for Japan Post Bank.
