本フォームは、学外研鑽プラスを利用した出張がキャンセルになった場合に限り使用してください。
本フォームからのキャンセル報告後に、キャンセル報告の取消しはできません。
キャンセルにあたり、物品等請求システムの入力等は、支援学生及び指導教員の責任において遺漏なく完了してください。
You can use this form only when you cancel the business trip with the Off-Campus Study Plus.
Once you submit this form, you cannot withdraw from the Off-Campus Study Plus Cancellation.
The supported student and his/her academic supervisor are responsible for completing all the other cancellation procedures, such as entering information into the Purchase Request System.

本フォームからのキャンセル報告後に、合格となった学外研鑽プラスを取消して支援額全額を引揚げます。本出張に係るキャンセル料は本プログラム予算から支出できません。
