• 振込先口座情報や通帳見開きページの写し・入国確認書類を提出できない場合は、理由及び提出予定日を必ず詳記してください。提出が無い場合で、理由の記載が無い又は不明瞭である場合は、申請を辞退したものとみなします。
• At the time of online application, all the four documents (1) Application Form ~ (4) Document confirming entry into Japan are required to be submitted. (A copy of relevant bankbook page and document confirming entry into Japan are required for those who are applicable.)
• If you are unable to submit bank account information, a copy of relevant bankbook page, or document confirming entry into Japan, enter the reason and estimated date of submission in the corresponding “Reason for Non-Submission” section of the form. If you do not submit any of the documents without entering such information, your application will be withdrawn.

入力前に申請者本人の通帳やオンラインバンクのスクリーンショットをご用意の上、通帳等を確認しながら口座情報を入力してください。
Before entering your account information, please have the applicant’s bankbook or a screenshot of the online bank and enter the information into this form while verifying the documents.

支店コード・口座番号は通帳の青丸で囲った箇所の情報を入力してください。
